General Office Policy

Your First Appointment

When you arrive in our office, our staff will greet you with smiling faces. Generally, your first appointment will take a minimum of 1 1/2 hours. If you have not received our forms by mail or downloaded and completed them at home, we ask that you arrive thirty minutes before your visit to complete our forms. Bring your insurance card and any referral forms with you so that we may copy them.

What to Bring

Scheduling Follow-Up Visits

We are an Advanced Access office. We only schedule patients up to 6 weeks in advance. We keep open appointments in our schedule each day to meet your scheduling needs. Our office is very good at meeting our appointment schedule. We ask that you arrive 15 minutes before your scheduled appointment time. If you arrive more than 15 minutes late you will be asked to reschedule your appointment.

We are very aware that patients dislike waiting long periods in a doctor's office for medical visits. While we try very hard to stay on schedule, sometimes delays can occur. Patients who are sicker or more complicated than we expected are often a reason. If you have waited in our waiting room more than 20 minutes please inform our front office staff.

Diabetic patients are encouraged to bring their blood sugar records to all office visits. If the patient is using an Accu-chek meter, bring the meter instead to the visit. We are able to download the information directly off the meter by computer.

Patients are always encouraged to bring a record of the medications they take or the medications themselves.

Lab Tests, Radiology and Ultrasound

After the first appointment, we usually order a variety of blood and urine tests to determine the cause of your symptoms. Please complete the tests at least two weeks before your next appointment so that our staff and doctors will have the test results in hand when you return. We normally direct patients to use Mid America Laboratories. They are located just down the hall from our office.

We recommend you always check with your insurance first to make sure any test you have done will be covered. Our office is not responsible for cost if you go to a facility that is not covered by your insurance.

When a radiology test is ordered that we do not perform in our office, you will be directed to call Centralized Scheduling at 317-355-4680 to schedule this test. Billing for these services will be done by the provider of service.

Insurance & Billing

We participate in a number of health insurance plans. Each plan has very specific referral and co-payment requirements. It is your responsibility to know what your insurance requires and it is up to you to fulfill those requirements. Your insurance company, not our staff, can answer your insurance questions.

You may view our Financial Policy online or obtain a copy from our office. Your signature on our registration page acknowledges that you will comply with our policy.

Our billing office is open Monday through Thursday from 9 a.m. to 4 p.m. Call 317-621-1017 and Claudia, our billing manager, will gladly assist you with any concerns you may have.
